All-Payer Claims Database
Compiling claims information from all payers can help states and their partners assess performance across systems and payers. It can give policymakers and other stakeholders actionable information to drive quality improvement and cost savings.

*Relevance to the ACA:
Information from an all-payer claims database can help states implement key aspects of the ACA successfully and assess the effect of the reforms. While developing a database is not required by the law, it may be advisable. For example, all-payer claims data may help:
Exchanges make informed decisions about whether a health plan ought to be allowed to participate (see §1311).
States determine whether a health insurers’ proposed premium increases are reasonable (see §1003).
Sections of the ACA that may help states build all-payer claims database include:
§3021 – Mandate of new Center for Medicare and Medicaid Innovation (CMMI) allows it to “test and evaluate systems of all-payer payment reform.” If linked with payment reform, state development of an all-payer claims database might be eligible for funding through CMMI.
§10332 – Medicare claims data to be available to states (and other entities) developing databases.

I wanted to share the hot-off-the-press MA Attorney General’s cost containment report, for which Freedman HealthCare provided consultative support on the analysis. In addition to its key findings (payment disparities persist, payment does not relate to quality of care, global payment hasn’t saved any money, and disparities in spending exist by ZIP code with poorer areas subsidizing the more expensive care of wealthier ones ), there are also references to opportunities for improvement including the broader use of APCD data and standardized quality measurement. Some of the lessons learned in MA certainly can be applied globally across the country.
West Virginia
The APCD White Paper served as the basis of information in passing APCD legislation in the 2011 WV Legislative Session.
West Virginia has created in statute an All Payer Claims Database. The APCD Act creates three MOU parties in State government. These entities are tasked with developing collection; storage and retention; release; and analytical rules that must be approved by WV Legislature.

The Iowa Legislative Health Care Coverage Commission presented 18 recommendations to General Assembly regarding implementation of the ACA. Highlights include planning for a new eligibility system to support Medicaid, CHIP, implementing a number of cost-containment strategies, and establishing an operational insurance information exchange by July 1, 2011.
